I was challenged recently for questioning the claim that G-d controls everything that happens in the world. Please produce where Chazal or Rishonim make such a claim Where does Ramchal or Rav Dessler state such an idea?
What they mean is that for most people, Hashem's hashgacha operates through what appears to be the ordinary course of nature. The Divine guidance is present, but it is concealed within the normal workings of the world and often goes unnoticed. Events generally unfold in a way that seems natural and predictable to the average observer.
If He created the world and His bestowal of Divine energy is what sustains it to the point that if He were to pause that bestowal for an instant everything would completely disappear then yes, He controls everything that happens in the world since nothing happens without His energy allowing it to.
depends what he means by נסים . Why did he leave Spain? Was it solely to escape the edict of the King after the disputation, or was it purely his desire to fulfil the mitzvah of living in E'Y? If he left because of the edict, which was to save his life from the church and the King, then he is reacting to teva in a rational manner. If he believed everything is miracle, then he had nothing to fear from the teva. Besides this, it is anti-halacha. Why does the Talmud forbid us to walk beneath a weak wall? It is about gravity, the bricks could collapse and endagener your life.
The clue is that the rabbonim who shout the most about how everything is from Shamayom and the doctors and scientists don't know anything are regulars in medical clinics and hospitals anyway. Besides, if Torah protects, why are there Shomrim?
There are things that are מנהגו של עולם they may also be nissim. EG a boy is very good at sciences at school, but not good at literature. He becomes a successful scientist or doctor - it is a ness in the sense that G-d has given him these talents. But he still has to follow the way of the world, study hard, learn how to do good interviews to find a job and parnassah, and not think he's going to be a businessman or diamond dealer. Same goes with rambam and Ramban who were both professional doctors. And btw they had to study their field with secular books. Of course it's a ness that after Rambam flees Spain he finally finds a job as the doctor of the Egyptian Sultan - although this limits his learning time to only on shabbat.

Nisim here means hashgacha pratis (as opposed to mikreh). This doesn't preclude hishtadlus
