I see this a shocking wake up call. Serious stuff. Here is the website of the consulate in Milan, just to give an example, where we clearly see the normative practice is to refer to it as Milan, Italy.

Though the property has been owned by a Sephardic Jewish organization for over 120 years, Arabs were placed there by Jordan following its conquest of Jerusalem in 1948..."
